China is in the news for manufacturing fake toothpaste. Look in the news as it is causing issues similar to what you have. While this may not be sole cause of your problem, it may be contributing.
Read the label on your Colgate and see where it was made. You may want to toss that and get Tom's of Maine or Jason toothpaste. Throw your brush away also and get a new one.
