I have been faced with many cases like the one you are describing and it is difficult to determine the exact source of your sons infection via internet. However, it is important to take into consideration all factors that may be involved in the skin condition you have described. You may consider consulting a physician personally with your own conserns along with any suggestions i might help you with. In children with Down Syndrom several skin conditions may arise as they progress in age and mature. Although there are many that may be linked to what you have described consider researching further, Atopic Dermatitis, this is red, scaly, itch skin most likely to appear on the cheeks, behind the ears, knees or on elbows. However, it is not uncommon to find such things on feet and ankles. The way you described its \"mosquito\" looking rash leads me to believe that it is not this. Although it may seem dry and itchy Atopic dermatitis is a rash that spreads converging with other breakouts. If the rash seems as though they are isolated bumps it may be something else. Excema is a serious condition that may last a long period of time, espically in a young child. The following is a link to a picture of excema, (the worst i have ever seen it). I hope this might help you to better understand what excema looks like and possibly narrow your options. http://tray.dermatology.uiowa.edu/Images/Clin/AtopDerm004.jpg

good luck, i wish you the best.
